
Employment Application Instructions
To apply for a job at Mille Lacs Health System please follow these instructions. You must complete all three steps to be considered for a position. Please read all instructions before starting your application process. Links to each step are provided below the instructions.
Step 1 - Create your user account
To submit an application form, you must first have a website user account. To create your user account:
- click the "create a user account" link below and you'll be directed to the User Registration page
- enter a user name
- enter a valid email address
- then click the "Create new account" button
A confirmation email will be sent to your email address. Then you-
- click the login link supplied in the email and you'll return to the website user page
- enter a new password for your account
- this validates your user account
- use your user name & password to login anytime to edit your application form or to apply for jobs
Keep a record of your login info in a safe place.
Step 2 - Complete your application form
A completed application form is required to enable the job apply process. Once you have a completed form, you can use that to apply for positions at Mille Lacs Health System. You do not have to fill out another application form to apply for another job.
- click the "create your application form" link below
- be sure to respond to each required field (marked with a red star *)
- complete other fields as appropriate for your situation
- if you have one, you can attach a resume document in either PDF or Word format. Click the "Resume Attach" link to access the upload field
- carefully read the "Signature" section and click the box to indicate you agree to the terms
- click the "Submit" button at the bottom to save your application form
When you submit your application form, you will be sent an email confirming the receipt of your application form. Included in the email is a link to your unique application form which allows you to update your information at any time. Keep a record of the link info, along with your username & password, so that you can access your application later.
Step 3 - Apply for a job
Once you have completed an application form, you will be able to apply for jobs. To select a job and apply:
- use the link below to view the list of jobs
- use the "Search Job Openings" form to search the list of current jobs available
- click on a job title to see more info
- on the job detail page, you'll see a "Click to apply for this job" link at the bottom
Once you have applied for a specific job, you will not be able to apply again for the same job, but you can use your same application form to apply for other jobs at Mille Lacs Health System.
When you apply for a job, the Mille Lacs Health System HR department is notified and will contact you as needed.
