We strive to make paying your bill as easy as possible. You may pay your bill online, send a check, or phone in a payment by calling 320-532-2640. We can also process payments in person should you choose.

We apologize, but due to a change in our billing system put in place to protect your privacy, you will not be able to make a payment through this site until you have set up new login information. This information will be located on your next statement, and it could be 4-5 weeks before you receive one of these statements. Yes, as long as we have your correct current insurance information. Without this information, you will be billed. Please bring your current insurance identification card to all appointments. You may also update your current insurance information by contacting the Business Office.

If I have more than one insurance plan, will you bill them both?

Yes, we will bill both plans, provided we have all the required information for both plans, including each subscriber’s name and date of birth so we can coordinate benefits correctly. The subscriber’s plan will be primary for him or herself, with the other plan as secondary.


You will receive a bill after we have filed the claim with your insurance(s) and they have processed it. The length of time this takes can vary.


We contract with outside providers to bring you certain services, such as some lab work, surgical services, and visits with specialists. As a result, you may receive a bill for their services from their office. If you have questions on these charges, please call the phone number listed on their statement.


Payments can be made by check, money order or credit card, by mail, or in person at any one of our clinics. Credit card payments can also be made online or by calling 320-532-3154, ext. 2640. Or, click here to pay online at MLHS. We accept Visa, MasterCard and Discover.
